Note:

You must be an Administrator to complete this function.

Select Care Plan and Letter Templates

Click the Administration Menu, then click Events, Organizations, Person, then click Care Plan and Letter Templates.

Create Form Letter

In the Create a New Template section →Template Type field, select “Form Letter.

Name Letter

Enter the form letter name and click Create.

Add Content

To add the form letter content:

Drag an existing formatted letter to the letter space and drop.

Or

Create a new letter from scratch. Add the form letter content to the letter space.

Add Merge Fields

Select the Reports Menu.

Click in the letter canvas. Place your cursor in the following areas of the letter to merge content.

Click Merge Fields then select “DeliquentImmunization”

and

“FullName”.

Create Template

Click Create Template to Save.
